Bệnh nhân nam trên 60 tuổi, khoẻ mạnh, không có tiền sử bệnh, nhập viện do viêm đường khí quản cấp tính (ARI)
– Ngày 1: bệnh nhân bị thiếu oxy, cần 35% oxy hỗ trợ (không khí bình thường là 21%). Bệnh viện cách ly bệnh nhân, xét nghiệm COVID-19, và chữa viêm phổi. XR ngày 1 cho thấy bình thường.
– Ngày 2 đến ngày 4: Bệnh nhân liên tục thiếu oxy, hỗ trợ 100% oxygen đường thở, CXR và CT cho thấy “mờ đục kính hai bên phổi vơí nhiếu miếng lát đen xen”. (hình 2 và 3)
– Ngày 5: BN có dấu hiệu lơ mơ, có thể do thiếu oxy. BS Pulido quyết định dùng Plaquenil khi thấy bệnh nhân không hiệu quả với trị liệu hỗ trợ và trụ sịnh. Liều 400mg tab 2 lần/ngày cho ngày đầu tiên, và 200mg ngày 2 lần sau đó kèm theo Zinc Sulfate. BS cần theo dõi EKG và G6PD. Sau đó, lab test Covid-19 là dương tính.
– Ngày 6: tình trạng tâm thần của bệnh nhân cải thiện, và oxygen giảm còn 50%.
– Ngày 7: Bệnh nhân có thể tự đứng lên trong phòng. Hiện giờ, sau vài ngày, bệnh nhân chỉ còn cần oxy 28% và khá hơn.

Hello health care colleagues, I wanted to share:

A older patient, otherwise healthy who denied risk factors associated with COVID presented with an acute respiratory illness.

– On day 1, he was moderately hypoxemic (low oxygen levels) requiring 35% low flow oxygen therapy and we decided to isolate and test COVID on presentation and treat for possible bacterial pneumonia.

– From day 2 through 4, his respiratory status declined and his Oxygen requirements increased to 100% high flow oxygen therapy.

– CTA chest scan was performed and show “peripheral ground glass opacities with crazy paving” which has been reported as COVID radiographic findings.

– On day 5, he started showing signs of delirium, likely due to severe hypoxemia. I decided to use Hydroxychloroquine “plaquenil” due to lack of response to antibiotics. I started with Plaquenil 400mg tab BID x 1 day then 200mg BID afterwards with Zinc sulfate. FYI.. need monitoring of daily QTC and G6PD (side effects)

On Day 5, His COVID-19 came back positive. On Day 6, his mental state improved and oxygen therapy was able to be reduced to 50%. On day 7, he is now walking around the ICU room getting his physical and mental strength back. Currently he’s only on 28% oxygen, (room air is 21% Oxygen) and states feeling better.
